INTRODUCING THE COLORADO AUTOMATED PAYMENT CARD!

UNEMPLOYMENT INSURANCE BENEFITS DEPOSITED TO THE COLORADO AUTOMATED PAYMENT CARD!

All unemployment insurance (UI) benefits are deposited to the Colorado Automated Payment (CAP) Card, a Visa® debit card provided by Chase Bank (Chase).

CAP Cards are mailed only to claimants with active UI claims. If you file a new claim or reopen an existing claim, you will receive your CAP Card within five business days from the date your new claim is filed or your existing claim is reopened. If you have an active UI Claim and have not received a CAP Card, contact Chase Customer Service at 1-866-316-3925 (toll free).

A CAP Card will not be mailed to you if you returned to work, canceled your claim, or stopped requesting the payment of benefits on the Colorado Unemployment Benefits Line (CUBLine) or CUBLine Online. If you have questions about the status of your UI claim, call the UI Customer Contact Center at 303-318-9000 (Denver-metro area) or 1-800-388-5515 (outside Denver-metro area).

Reminder notices informing you of dates to file for payment via CUBLine or CUBLine Online are not mailed. You are responsible for knowing when to request payment on CUBLine or CUBLine Online.

Your CAP Card will arrive from Chase in a plain, white, window envelope. Do not throw away this envelope without removing its contents. Once a CAP Card is issued, it is valid for three years. Keep your CAP Card until the expiration date for payment on a potential future benefit claim.

As a general practice, do not give out any information about your account number, expiration date, or account balance to anyone who calls you on the telephone. Protect your CAP Card as you would credit cards or cash. Never give your UI or CAP Card personal identification number (PIN) to anyone. To access more information on steps to safeguard your card, please click on Important Information for Safeguarding Your CAP Card.

As soon as your CAP Card is received, sign it and call Chase Customer Service at 1-866-316-3925 (toll free) to activate your card and to select a CAP Card PIN. Carefully read all of the enclosed information, which includes:

For questions about your CAP Card account, contact Chase Customer Service at 1-866-316-3925 (toll free) or visit their Web site at www.myaccount.chase.com. The UI Customer Contact Center cannot answer questions about your CAP Card account.
