The Colorado Department of Labor and Employment, Unemployment Insurance (UI) Program will be mailing the income-tax Form 1099-G, Certain Government Payments, on January 16, 2008, to all claimants that were issued UI benefit payments between January 1, 2007, and December 31, 2007. If you do not receive Form 1099-G in the mail, contact the UI Customer Contact Center, but please allow four weeks from January 16, 2008, before requesting a duplicate copy of the form.

OVERVIEW AND ADDRESS CHANGE

Form 1099G, Certain Government Payments, is mailed in January to anyone who received an unemployment insurance (UI) benefits payment during the previous calendar year.

If your Form 1099G is mailed to an address other than your current address, the U.S. Postal Service returns Form 1099G to UI Operations; the U.S. Postal Service does not forward Form 1099G to your current address. UI Operations keeps the returned Form 1099G and automatically remails it once you submit an address change.
