To Report Unemployment Insurance (UI) Fraud: 

To provide information about an Unemployment Insurance (UI) claimant receiving benefits inappropriately (i.e. working while collecting benefits), Click here to submit an email answering the following:

  1. Who is committing Unemployment Insurance Fraud?

  2. What is their street address?   
  3. City, State, Zip  
  4. What is their telephone number?
  5. What is their Social Security number?     
  6. What is their date of birth?
  7. Explain briefly how they are committing fraud:
  8. When did the alleged fraud start?
  9. If they are working, what is their employer's name, address and telephone number?

We will review the necessary files and records in light of the information you provided, to determine the most appropriate action.  If you provided information about yourself, you will be contacted again only if it is necessary to complete our review or investigation.
